Through an innovative, community-based model of integrated care for vulnerable populations, Bridge to Health Primary Care delivers high-quality healthcare alongside a comprehensive network of wraparound services that address the social determinants of health. These include targeted support for housing stability, economic mobility, food security, mental health, family stability, public benefits navigation, and the built environment.
Embedded within La Colaborativa’s Survival Center, a trusted hub of community resources, Bridge to Health Primary Care is intentionally designed to remove barriers to healthcare access, provide leading-edge primary care, and connect patients to the supportive services they need to improve their health, strengthen their stability, and enhance their overall well-being.
Requirements
If you are 14 years or older, you may qualify if all of the following apply to you:
- You live in Chelsea, Revere, East Boston, Everett, or Lynn
- You are a member of La Colaborativa
- You do not have health insurance, or only have MassHealth Limited.
- Your household income meets the program guidelines (less than 300% of the Federal Poverty Level). If you’re not sure, our team can help you find out!
Your Benefits
- Unlimited Care: including in-person visits, phone consultations, video appointments and support through the Liza app.
- Bilingual Support: Our care team speaks English and Spanish, and can assist in other languages through interpretation services.
- Flexible Appointments: Walk-in and scheduled appointments are available Tuesday through Friday. Virtual visits are available on Mondays and between in-person appointments.
- Affordable Labs and Medical Services: Patients have access to discounted laboratory tests and medical services through our trusted community healthcare partners, making care more affordable and accessible.
- After-hours phone or Liza app: Health concerns do not always happen during business hours. You can always ask questions to Liza App or call us!
*Our team is available 24/7. Our after-hours answering service provides language support and connects you to La Colaborativa’s wraparound services, including case management, food assistance, housing support, employment resources, and more. We are here to help you and your family access the support you need, whenever you need it.

Meet Dr. Diana Ramirez

We are thrilled to welcome Dr. Diana Ramirez as the new primary care physician at Bridge to Health Primary Care. A board-certified family physician with over 20 years of experience, Dr. Ramirez specializes in preventive care, chronic disease management, women’s health, and mental health. As an immigrant herself, she is deeply committed to providing compassionate, culturally respectful care to every patient in both English and Spanish. Supporting Dr. Ramirez and the rest of our dedicated team is Liza, our 24/7 virtual health assistant and your digital health guide. Powered by advanced technology, Liza is always available to answer general health questions, offer wellness guidance, and help connect you to the services and resources that best meet your needs.

- No bills for primary care visits.
We also help connect you with programs that reduce the cost of medications, lab testing, and specialty care when needed.
Can I receive care if I am undocumented or do not have immigration status?
Yes. Your immigration status does not affect your ability to receive care at Bridge to Health Primary Care. We will never ask about your immigration status, and your privacy is always protected. Everyone who meets the eligibility requirements is welcome.
Is my information private?
Yes. Your medical information and messages through the Liza app are confidential and protected.
How do I register?
- Become a La Colaborativa member (if you are not already).

- Complete the onboarding process with La Colaborativa’s team.
